Contracture Patient 01
Description:
38 yr. 5’ 3” 112 lb. lady presented to Dr. Pertsch 1 yr. 3 mo. after having subglandular breast augmentation saline 210 c filled to 240 cc bilaterally. She went from 34A to a 34C. Also had a left breast biopsy in the past.
Overall she was happy with her size. Fullness in the right upper breast never settled. She developed firmness on both sides (capsular contracture) not helped with a closed capsulotomy (where her doctor vigorously squeezed breast to break capsule and fortunately not break implant). Right implant farther off to the side. Subglandular pocket almost crossed midline joining both breast pockets (i.e. almost a synmastia). Although she has slight rippling on the inner breast that would have been reduced with gel implants she preferred “safety” of saline implants.
Dr Pertsch’s revision surgery consisted of complete capsulectomy on both sides (28.4 gm. right and 14.2 gm. left was tissue removed)
Old implants removed and discarded.
New implants placed under pectoralis muscle (dual plane)
Used original periareolar incision, and revised both incisions
New saline smooth implants 240cc filled to 270cc on both sides. (increased volume offset capsule tissue removed)
After photos taken 12 years after Dr Pertsch revision surgery. (although hair style so different patient: mole pattern under R breast confirms same patient!)
She is now 5 5’3” 132 lb. was 36 now 38B
She had two more breast biopsies
Breasts feel soft and natural when touched.
Really loves them as they have settled a bit.
